Observation

(2a) In the infant room there are staples on the wall directly adjacent to the cribs within a child's reach. This poses a risk of choking. (2b) In the infant room the horizontal blinds are screwed into the lower sill. There are still center cords that are capable of forming a loop. (2c) In the toddler room there are loose cords accessible to children against the wall in an opening in the storage shelf. (3a) Pre-school room: the front of the storage cubbies are splintered. (3c) Infant room: Bi-fold closet door has come off the track and could pinch fingers. Outdoors, there are missing or broken pieces of toys: Orange car with large cuts in two back wheels, blue police boat with a handle missing, basket ball stand and hoop is broken with sharp edges. Metal tunnel and the princess castle have sharp edges and broken parts. There are loose retaining wall bricks in danger of being dislodged. (3g) The blue napping mats appear dirty or stained and in poor condition. (4c) Lighting safety: Ceiling lighting in the infant room does not have shatter resistant bulbs or covering. Table lamp in the school age room has an uncovered bulb that is not shatter resistant. (4f) Safety gate at the top of the stairs was not in place during the visit. (5a) Oulet on the floor in the lobby is not tamper resistant. Outlets in the school age area of the facility are not tamper resistant. (5d) String lights in the preschool classroom were not plugged in causing a hanging cord. (5e) Surge protector and cords accessible under the computer table.
